- National Statistics Office (NSO) or Philippine Statistics Authority or PSA Birth Certificate
- NSO or PSA Marriage Contract for married female applicants
- Transcript of Records (TOR) with scanned picture and Special Order No. and remarks “For Board Examination Purposes”
- valid NBI Clearance
According to PRC, if you failed the Dentist Board Exam for three (3) times already, you need to take a refresher course. The said rule was effective May 2007.
